Required documentation for a medical withdrawal

When pursuing a Medical Withdrawal (or a compassionate withdrawal due to another individual’s medical situation), it is necessary to include official documentation in the form of a letter from your attending health care provider typed on official letterhead. The letter will need to include the following information:
- The date of onset of illness.
- The dates you were under professional care.
- The general nature of your medical condition and why/how it prevented you from completing your course work/attending class (specific to requested semester).
- The date of your anticipated return to school.
- The last date you were able to attend class.
- For a partial medical withdrawal — the letter should justify the selective nature of withdrawing from some but not all classes.
Note: The withdrawal designee may seek additional information from you or from those providing the documentation. Verification of authenticity of documentation is standard practice. All supporting documentation should be detailed and relevant to the situation.
